Incisional Refractive Surgery or Radial Keratotomy consists in performing incisionin a radial form, such as bicycle’s wheel, it usually takes 4 to 8 incisions using a diamond blade. These incisions weaken the peripheral portion of the cornea, which yields to the intraocular pressure to bulges; the central portion then flattens correcting myopia and stigmatism. The length, deepness and number of incisions are based in the quantity of diopters to be corrected

 

History

Radial Keratotomy (RK) was the most used technique for the correction of myopia and astigmatism between 1975 and 1995. Over one million people between the ages of 40-70 have had RK.  The results of RK were mainly successful in the vast majority of cases. However, over the years, patients develop presbyopia (the loss of near vision due to aging), and in some cases, irregular astigmatism due to the pronounced scarring of the incisions.

 

Manifestations

Many of these patients who at the beginning reported good vision, refer now a decrease in the quality and the quantity of their vision, and mainly having difficulty correcting their visual acuity, which at times does not improve with glasses or contact lenses, due to the irregularity of the cornea secondary to the incisions, and in some cases, patients low tolerance to contact lenses. 

 

Therapeutic Options

CODET has developed different techniques to counteract and improve the vision of patients that received the RK procedure several decades ago:

 

  • For higher prescriptions, relying on modern versions of the Staar Surgical Implantable Contact Lens (ICL)

 

  • Finally, for those extreme cases where the scar tissue of the incisions has created irregular astigmatism and the patient cannot tolerate a special contact lens therefore, both the visual acuity and quality of vision have decreased, CODET has developed a unique and innovative Deep Lamellar Keratoplasty (DALK) technique assisted by the Ziemer LDV Z8 Femtosecond Laser, allowing for an excellent recovery of both, visual acuity and visual quality.
